On January 5, 2015 an employee was walking along the roof area where other contractors had been installing structural steel beams. He lifted an opening in the floor and fell through the hole to the ground. He received multiple abrasions to his head, arms, and other parts of his body.
Proper flagging/barricading could have prevented the employee from entering the area.
What Can We Learn?
A Questioning Attitude could have easily prevented this incident from happening.
Effective Communication between the employee and the contractors could have helped identify the hazards.
His Overconfidence caused him think he could open the hole without falling through.
