Fall Protection (KV93)


What Happened?

On August 12, 2012, an employee was applying exterior stucco to a single-family residence that was under construction. He fell through the gap between the structure and the inboard edge of the scaffold. The employee fell approximately 12 feet to the ground and suffered abrasions and contusions. He was transported to the hospital and was kept overnight for observations.

Contributing Factors

Using a 2-Minute Drill would have helped the employee identify all of the necessary PPE for fall protection.

What Can We Learn?


The employee should have used a Questioning Attitude before getting on the scaffolding without fall protection. 


The employee was Overconfident in his daily task. This was not the first time he entered scaffolding with open gaps at elevated heights.
