Grinder Injury

What Happened?

A worker received a significant head injury when using a grinder during the repair of a tank. While conducting grinding activities, the draw-string from the hood of his jacket became entangled in the wheel of his grinder causing it to be pulled towards his head. He received cuts to his temple and neck, requiring off site treatment.

Contributing Factors

There should be no drawstrings, long hair or loose items of clothing dangling when operating a grinder or other rotating equipment.

What Can We Learn?


Perform a Self Check to ensure there was nothing dangling that potentially could get caught in the equipment. 


Vague Guidance, due to a lack of details in the procedure regarding restrictions of loose clothing while operating rotating equipment.
