Face Laceration


What Happened?

June 21, 2013 An employee was operating a wire rope winding machine. As he finished up winding the spool he went to secure the end of the wire rope with a staple. The staple came loose allowing the spool to unwind. The end of the wire lacerated the employees temple and forehead.

Contributing Factors

A face shield was required while performing this task but was not worn.

What Can We Learn?


Using a Questioning Attitude would have helped the worker consider the worst thing that could happen and also driven him to obtain the appropriate PPE for the task.

The Procedure states a face shield is required while performing this task.


The employee was a tenured employee who had done  the job a hundred times before. His complacency allowed him to fall into the Overconfidence trap.
